Portville Home Care Services offers Residential Habilitation Services that are structured to give each person the professional services required to meet their goals and accordingly satisfy their plan of care and individual support plan.
As part of our Home and Community-Based Services for Adults with Developmental Disabilities (HCBS-DD), these habilitation services are designed to acquire, retain, and improve the self-care, daily living, adaptive, and other skills of the developmentally disabled as needed to successfully reside at home within the community.
