Portville Home Care Services provides one-on-one services to individuals with intellectual and developmental disabilities who are in need of support when accessing various resources in the community. This program enables the individuals we serve to thrive in the community and to live an independent life.
Community Connector Services include:
- Supporting the participant’s abilities and skills necessary to access typical activities and functions of community life
- Utilizing the community as a learning environment in order to assist the client in building lasting relationships and natural support in the residential community
The Community Connector Services are provided to a single participant in various settings where they can interact with individuals without disabilities. Parents and/or family members are unable to perform this service.
