We help individuals with developmental disabilities and their families residing in Colorado receive the appropriate care and support for their condition. Through this service, the developmentally disabled population is provided with the appropriate support and resources that will allow them to bolster their health, independence, and overall wellness while upholding their dignity and integrity.
Portville Home Care Services’ is in partnership with the following organizations and agencies:
- North Metro Community Services
- Developmental Pathways (DP)
- Developmental Disabilities DBA IMAGINE
- Rocky Mountain Human Services (RMHS)
- Developmental Disabilities Resources Center (DDRC)
CCBs can be provided to eligible residents of Colorado under the HCBS Medicaid waivers, such as SLS waiver and DD waiver.
